How to File a Claim


Claims may be submitted using an online form in the Williams Emtal Talc Lucid Portal.

If you are a Claimant (not associated with a law firm) logging into the portal for the first time, you will be asked to create an account. After you have registered an account and created your claim, you will also be able to access your claim through the portal to provide supplemental information if necessary during the claim review process.

Law firm users must register with the Settlement Administrator and execute and Electronic Filer Agreement to gain access to the portal.

Electronic versions of all of the forms listed below will be available when completing your claim in the portal. However, each claim must include a Verification Form that is signed by hand.

Please follow the filing instructions carefully, fill out the form(s) completely, and attach the required supporting documentation to ensure timely processing. All Claim Submissions must be postmarked no later than 11:59 P.M. January 15, 2021.

If a law firm represented you in your lawsuit, they may also be able to assist you with your claim filing.

If you are not sure whether you are a Class Member

If you think you may be a Class Member, but do not have documentation to support your claim, you can ask the Settlement Administrator to search the Underlying Lawsuit archives for documents related to your case. You may submit a document search request in the Williams Emtal Talc Lucid Portal or request assistance by contacting the Administrator.
